Permit application
A permit application must be submitted in writing to the Swedish National Institute of Public Health by anyone handling the substances listed in the appendix to the Ordinance regarding a Prohibition of Certain Goods Dangerous to Health (1999:58) for scientific or industrial purposes. The permit is valid for one year.
The application must include:
- information on the applicant’s name, full address, telephone number and possible e-mail address
- a copy of the registration certificate from the Swedish Companies Registration Office (corporate ID number)
information on the street address of the facilities used for the handling of the substance(s)
- information on which substances the application concerns
- and an estimate of the amount that will be handled in the upcoming 12-month period, as well as a compilation of the amount handled in the previous 12 months
- information on any product name(s), declaration of contents
- and the area(s) of use
- the signature of the company’s authorised signatory

Changed conditions
Anyone conducting activities requiring permits under the Ordinance regarding the Prohibition of Certain Goods Dangerous to Health must inform SNIPH of changes to the conditions stated in a permit application without delay.
